Students using a proctor for exams (only out-of-area students) need to submit a signed proctor form (see Distance Learning section on Taft College website) to the instructor at the beginning of the course. If the instructor does not have a signed proctor form BY October 9th, the exam must be taken in the Library on the Taft College campus. This is to allow time for the instructor to contact the proctor for approval (this is done by email, so have an accurate email address for your proctor on the form), and then mailing the exam to the proctor if they are approved. Students must be self-motivated and work independently to be successful in this course.
  • Assignments & Tests: Students will be responsible for the following assignments, projects, papers, and exams:
    • Assess Yourself Questionnaire This project is done online. 50 points possible. Due October 26th.
    • Behavior Change Contract This project is done online. 50 points possible. Due October 26th.
    • Food Journal with evaluation (see Chapter 8) -This project is submitted online. 50 points possible. Due November 6th.
    • Family Health Tree project Due December 4th, 100 points possible.

You may use the example chart from the link, or you may develop your own chart. On question #3, list all ages of the living family members, including you, regardless of whether or not they have any current health issues. You will enter a statement in the assignment area that you have either mailed this document to me or that you have dropped it off in the Library on campus.

When mailing this project to me, IT NEEDS TO BE POSTMARKED BY THE DUE DATE TO AVOID A LATE PENALTY.

    • Exam 1 - to be taken on Tuesday, October 20th, Chapters 1-5, 100 points possible.
    • Exam 2 - to be taken on Thursday, November 5th, Chapters 6-10, 100 points possible.
    • Exam 3 - to be taken on Monday, November 30th, Chapters 11-15, 100 points possible.
    • Final Exam - to be taken on Monday, December 14th, Chapters 16, 17, 19, and 20. Skip Chapter 18. 126 points possible.
    • I do not give online exams in this course.

  • Proctored Exams: Yes, for non-Taft area residents. If you are using a proctor, it is your responsibility to provide that proctor with  4-9" X 12" manila envelopes, with postage attached, to mail your exams back to me. Taft-area residents are expected to take exams in the Library on the TC campus, proctored by college staff.
  • Additional Comments: Additional Comments: Because this is a distance learning course, attendance is not an issue. Turning assignments in on time is (late penalties are severe). The due dates for all projects are listed both on this Orientation sheet and the course Syllabus. If you do not meet the first assignment deadline, or take the first exam, I will assume you do not wish to stay in the class and drop you unless you contact me. After that, it is your responsibility to drop the course. Drops will not be backdated. Contact the counseling center or the distance learning help desk (661-763-7812) to submit drop slips. Here are some helpful suggestions for successfully completing the course:
    • Follow the schedule on the syllabus.
    • Read each chapter, and then create a set of notes to study.
    • Study all terms -- any of them could appear on an exam.
    • Do any of the review activities that may help you at the end of each chapter. These do not need to be turned in to me.
    • Turn in assignments and take exams on the date listed -- all late work is docked (10% the 1st week, 20% the 2nd week) and not accepted at all if more than two weeks late.
    • Exams are a combination of multiple choice and true/false.
    • STUDENTS NEED TO USE SCANTRON SHEETS ON EXAMS -- PREFERABLY FORM #882-E, AVAILABLE IN ANY COLLEGE BOOKSTORE.
